Evidence of identity 

You must complete an evidence of identity (EOI) form and present original documents showing your name, photo, date of birth, signature and current address to the value of 100 points to the trainer or assessor before:

  • undertaking an assessment for a national licence to perform high risk work (high risk work licence)
  • attending OHS construction induction training
  • attending a OHS consultation course.

Commonly used combinations of evidence of identity documents include:

  • an Australian birth certificate and an Australian driver's licence, or
  • an Australian drivers licence, Medicare card, ATM card and a utilities bill.

It is the responsibility of the trainer or assessor to verify the documentation and ensure the correct form is completed. The form is to be retained by the Registered Training Organisation.


 

Special provisions

Special evidence of identity provisions may apply to :

  • OHS construction induction training for Aboriginal and Torres Strait Islanders, school students, overseas persons who have been in Australia for less than six weeks and correctional centre inmates
  • training and assessment for high risk work licences for Aboriginal and Torres Strait Islanders and correctional centre inmates.
